Decision signals and actions
Score these, not a blog-grade star rating:
- False auto-approve on a planted sole-source canary
- False-hold rate (throughput)
- Disagreement after a policy-text edit
Pair auto-act errors with handoff rate. If the approvals orchestrator never acts, you have not evaluated finance approval decisions — you have evaluated a human queue.
